When enrolling your child in the Preschool or Childcare Center, it is important to make sure all the following documents are completed and provided back to the Coordinator. Below we list the forms in which should be completed. You may find the documents to download and complete in the section below.

Preschool:

• Physical

• Immunizations (Current and signed by a nurse)

• In case of Emergency form

• Permissions form

• Student Vision Card

• Withdrawal Form

• Statement of Health (ages 5 and up)

• Exemption Form (if application)

Childcare:

• Physical

• Immunizations (Current and signed by a nurse)

• Child's Development History

• In case of Emergency form

• Child Release Form

• Application for Free & Reduced-Price Childcare Meals (if application)
